The information from the Treatment Cost Estimator provides only an estimate of the out-of-pocket costs you may incur for your medical treatment. It does not reflect the costs for which you may be responsible to specific providers of health care services. When looking at what’s included in the detailed out-of- pocket costs for a specific medical condition, the column titled “est. paid by your health plan” reflects the amount covered by the health plan, not necessarily what the plan will pay.
